Hello and thank you for your interest in creating a DA document.
We have recently changed the way DAs are written, submitted and reviewed. Please ensure you read this page fully to avoid rejections of your submitted DA.
Please be aware that, as of the 3rd of May 2026, DA submissions must no longer include a titled addendum or incident log. Refer to the image below.
DA Submissions are purely drafts of what we will publish on the database. The submissions must follow and adhere to the criteria outlined in this document and must be submitted via this document. The completed template must be sent to codacontainment@gmail.com or handed to the Documentation Department.
